Appointments

Please download the referral form below, simply fill out the form with signature and email to enquiries@radpidiagnostics.com.

Once the request form is received and accepted, we will contact the patient or nominated representative with appropriate invoice if applicable, followed by appointment details using the information provided.

Fees for Private home x-rays or home ultrasound depends on the location (travel time) and body parts involved. Additional body part X-rayed / scanned on the same visit cost 60% less. Images are taken in the property of the client’s.

Send the ‘referral form’ to enquiries@radpidiagnostics.com. Referral forms will be vetted by our healthcare team. Once accepted, an appropriate invoice will be issued to the patient or nominated representative.

Payment of invoice is made using a link or bank details provided. Appointment will be issued after payment. Please note that Radpid Diagnostics expect the patient’s nominated carer or helper to be around during the examination.
